The creator begins by showing the land where there was previously only an empty lot filled with mobs and explains that his goal is to open the first Brainrots Hospital on the server – a safe haven to cure zombified villagers and infected players. After a brief introduction, he presents the building's model in creative mode and details the key blocks: plenty of quartz for the walls, stained glass for the isolation windows, and waxed copper for the roof shaped like a giant red cross that serves as a nocturnal reference.

Next, he records the entire farming and construction process in survival mode. He gathers quartz in the Nether, trades glowstone for emeralds with clerics, smelts sand into glass, and uses tons of white wool for the beds. He installs automatic doors with slime blocks, lights that turn on with daylight sensors, and an alarm system connected to observers that triggers if a zombie crosses the entrance. He also creates two internal floors: on the lower floor, he places reception, a waiting room, and consulting rooms; on the upper floor, there are operating rooms, an ICU, and a weakness potion + golden apple lab.

During the construction, he receives visits from several companions from the Brainrots SMP. Amid jokes, one changes the sign to “Hospital de Brorans,” another unleashes a “raid” of baby zombies to test the defenses, and a third gifts him skeleton heads to decorate the X-ray room. There are tense moments when night falls and a horde breaches the perimeter; however, the alarm system works, and they manage to contain the infection using dispensers with splash potions.

In the final stretch, he places the last paintings, takes inventory of medical supplies, cuts the inaugural ribbon, and officially opens the hospital. He announces a challenge for the audience: if the video reaches a certain number of likes, he will add a pediatric wing and a helicopter pad for emergencies. He ends the episode by thanking the support, reminding new viewers to subscribe, and teasing that in the next installment he will build an automated pharmacy to supply medicines to all of Brainrots
